DescriptionTwo-page questionnaire issued by the War Department during World War II. The questionnaire is a typed mimeographed form with 28 questions. On the first page, the word "Confidential" are stamped in red at the top and bottom. Staple in top left corner. Questions range from questions about birth date and address to membership in organizations and the request for five references. Questions 27 and 28 are known as the loyalty questions. These two questions were designed to test the loyalty of alien and citizen Japanese Americans in the World War II concentration camps. Pasted to the bottom half of page 4 is a form asking the signee to choose whether he is willing or unwilling to serve in the United States Armed Forces.
